Survey Library—Create a New Customized Survey Category
Add a New Category
In this example, you will add a new category and items to the survey area Management Skills.
1. At the Management Skills survey area screen, click the + (Add) icon or click, Click here to create a NEW category.
2. At the New Category box type the name of the category, in the example above it's Employee Training Opportunities.
3. Add a Description (optional).
4. Click Save or press Enter. The category name will appear on the left.
Add a Structured Question to a Category (Scaled Input and Structured Comments)
1. Click the category name that you'd like the item to be under, in the case above it's Employee Training Opportunities.
2. At the Selected Category box, click Add Item.
3. At the New Item box, type the content of the item. In the above example it's Arranges opportunities for team members to cross-train.
4. Answer the question, What type of item is this? Use the default setting, Scaled with comment(s).
Structured question
Scaled – permits scaled ratings only
Scaled with comment(s) – default setting permits scaled and comment responses
Structured comments only – permits comments only, no scales responses
5. Click Save or press Enter. The item appears in the category folder on the left.
6. Repeat steps 1-5 to add additional items.
Add an Open-Ended Question to a Category
1. At the Selected Category box, click Add Item.
2. At the New Item box type the content of the item. In the example above the item is What additional comments do you have regarding employee training opportunities?
3. Answer the question, What type of item is this?
4. Choose Open-ended question (text-only response).
5. Click Save or press Enter. The item appears in the category folder on the left.
Notice that open-ended questions have a different icon from other items.
6. Repeat steps 1-5 to add additional open-ended questions.
